Resumo Objetivo: Avaliar o astigmatismo induzido pela cirurgia de catarata através da técnica de facoemulsificação (FACO) guiada pelo planejador cirúrgico VERION®, em um serviço oftalmológico do Paraná. Métodos: O estudo tem caráter observacional com avaliação individualizada de prontuários de forma retrospectiva, analisando 37 olhos de 20 pacientes operados de catarata pela técnica de FACO com a utilização do VERION® e implantação de lente não-tórica no Hospital de Olhos de Cascavel - PR no período de maio de 2016 a novembro de 2018. Resultados: Dentro de nossa amostra composta por 37 olhos abordados com assistência do VERION®, 43% dos participantes do estudo (n=16) apresentaram redução do astigmatismo inicial, inclusive com eliminação de graus mais graves de astigmatismo (≥2.5 D). Conclusão: O impacto do VERION® foi significativo uma vez que permitiu a correção do astigmatismo prévio de uma parte da amostragem. Em olhos que ocorreram astigmatismo induzido cirurgicamente essa complicação foi menos relevante clinicamente em comparação com incisões corneanas da técnica convencional.


Abstract Objective: To evaluate the astigmatism induced by cataract surgery through the phacoemulsification (PHACO) technique guided by the VERION® surgical planner, in an ophthalmological service of Paraná. Methods: This is an observational study with retrospective individualized evaluation of medical records, analyzing 37 eyes of 20 patients who underwent cataract surgery using the PHACO technique using VERION® and non-toric intraocular lens implantation at the Hospital de Olhos de Cascavel - PR in May 2016. Results: Within our sample of 37 eyes approached with VERION® assistance, 43% of study participants (n=16) had reduced initial astigmatism, including elimination of more severe degrees of astigmatism (≥2.5 D). Conclusion: The impact of VERION® was significant as it allowed the correction of previous astigmatism of a part of the sample. In eyes that had surgically induced astigmatism, this complication was less clinically relevant compared to conventional technique corneal incisions.

ABSTRACT Purpose: To assess the frequency of corneal astigmatism before cataract surgery in a Brazilian sample. Methods: This clinic-based cross-sectional study was conducted at the Bonsucesso Federal Hospital, Rio de Janeiro, Brazil. Charts of patients who underwent cataract surgery over a two-year period were retrospectively reviewed, and preoperative keratometric measurements were collected and analyzed. Results: A total of 1707 eyes of 1045 patients were enrolled. The corneal astigmatism was less than 1.0 D in 971 eyes (56.9%), 1.0-1.99 D in 496 eyes (29.1%), 2.0-2.99 D in 157 eyes (9.2%), and more than 3.0 D in 83 eyes (4.9%). The mean corneal astigmatism was 0.92 ± (SD) 0.96 D (range 0 - 10.25 D). Conclusion: Over 40% of the patients undergoing cataract surgery enrolled in this study had more than 1.0 D of corneal astigmatism and may benefit from the use of toric intraocular lenses. These data can be useful for planning to make this technology available for patients.


RESUMO Objetivo: Avaliar a prevalência do astigmatismo corneano antes da cirurgia de catarata em pacientes brasileiros. Métodos: Este estudo transversal de base clínica foi realizado no Hospital Federal de Bonsucesso, Rio de Janeiro, Brasil. Os prontuários de pacientes submetidos à cirurgia de catarata durante um período de dois anos foram revisados retrospectivamente, e as medidas ceratométricas pré-operatórias foram coletadas e analisadas. Resultados: Um total de 1.707 olhos de 1045 pacientes foram incluídos. O astigmatismo corneano foi menor que 1,0 D em 971 olhos (56,9%), 1,0-1,99 D em 496 olhos (29,1%), 2,0-2,99 D em 157 olhos (9,2%) e mais de 3,0 D em 83 olhos (4,9%). A média do astigmatismo corneano foi de 0,92 ± (SD) 0,96 D (intervalo 0-10,25 D). Conclusão: Mais de 40% dos pacientes estudados submetidos à cirurgia de catarata incluídos neste estudo tinham mais de 1,0 D de astigmatismo corneano e podem se beneficiar do uso de lentes intraoculares tóricas. Esses dados podem ser úteis no planejar a disponibilização dessa tecnologia para os pacientes.

Abstract Purpose: To report the prevalence and analyse corneal astigmatism patterns in cataract surgery candidates. Methods: Researchers examined 2136 eyes from 1204 patients for cataract surgery from 1 January to 31 December 2016. All the candidates were evaluated with partial coherence interferometry (IOLMaster). The results were analysed statistically in relation to qualitative and quantitative variables. Results: The mean age of the patients was 71.9 years with a female predominance. Mean corneal astigmatism was 1.0 dioptre (D). Against the rule (ATR) astigmatism increased with age. Women presented with steeper corneas than men. In those patients who had both eyes measured, we found strong correlations between amount of right and left eye astigmatism, axis and keratometry. Overall, 39% of the eyes had corneal astigmatism > 1.00 D and 19% >1.50 D. Conclusions: A significant percentage of our population presents with corneal astigmatism > 1.00 D with a shift from WTR (with the rule) to ATR with increasing age. Our findings, in a population that has never been described before, will be helpful to surgeons to enhance surgical techniques, patients to improve visual outcomes, IOL (intraocular lens) manufacturers to make better designs and health agents to optimise resources.


Resumo Objetivo: Relatar a prevalência e analisar os padrões de astigmatismo corneano em candidatos à cirurgia de catarata. Métodos: Os pesquisadores examinaram 2136 olhos de 1204 pacientes a cirurgia de catarata de 1 de janeiro a 31 de dezembro de 2016. Todos os candidatos foram avaliados com interferometria de coerência parcial (IOLMaster). Os resultados foram analisados estatisticamente em relação às variáveis qualitativa e quantitativa. Resultados: A média de idade dos pacientes foi de 71,9 anos, com predomínio do sexo feminino. A média de astigmatismo corneano foi de 1,0 dioptria (D). Contra a regra (ATR), o astigmatismo aumentou com a idade. As mulheres apresentaram córneas mais íngremes que os homens. Nos pacientes que tiveram os dois olhos medidos, encontramos fortes correlações entre quantidade de astigmatismo nos olhos direito e esquerdo, eixo e ceratometria. No geral, 39% dos olhos tinham astigmatismo corneano >1,00 D, e 19% > 1,50 D. Conclusões: Uma percentagem significativa da nossa população apresenta astigmatismo corneano >1,00, D com uma mudança de WTR (a favor da regra) para ATR (contra a regra) com o aumento da idade. Nossas descobertas em uma população que nunca foi descrita antes serão úteis para os cirurgiões melhorarem suas técnicas cirúrgicas, os pacientes melhorarem seus resultados visuais, os fabricantes de LIOs (lentes intraoculares) criarem projetos melhores, e os agentes de saúde otimizarem os recursos.

RESUMO Objetivo: Investigar a associação da hipermetropia com ambliopia, estrabismo, anisometropia e astigmatismo. Métodos: A hiperopia foi classificada em Grupo 1: maior ou igual a +5.00D; Grupo 2: maior que +3.25D e menor que +5.00D, com diferença de equivalente esférico maior ou igual a 0.50D; Grupo 3: maior que +3.25D e menor que +5.00D, com diferença de equivalente esférico menor que 0.50D e Grupo 4: com equivalente esférico maior e igual a +2.00D. O Grupo controle pertencente ao equivalente esférico menor que +2.00D. Resultados: A presença de hipermetropia maior e igual a SE+2.00D foi significativamente associada à maior proporção de crianças com ambliopia (27,2 vs. 14,8%, OR = 2,150, p<0,001) e estrabismo (70,8 vs. 39,3%, OR = 3,758, p<0,0001. A presença de hipermetropia também foi significativamente associada à maior proporção de anisometropia nos grupos com hipermetropia maior e igual a SE+2.00 (29,1 vs. 9,9%, OR = 3,708, p<0,0001) e astigmatismo (24 vs. 9,9%, OR = 2,859 p<0,0001). Conclusão: A presença e magnitude da hipermetropia entre crianças foram associadas à maior proporção de erros refrativos e visuais, como estrabismo, ambliopia, astigmatismo e anisometropia.


RESUMO Objective: Evaluate ocular trauma cases related to falling in elderly patients e compare the prevalence and severity of the cases. Methods: A series of cases was made with 52 patients aging 60 or more within the period of 36 months presenting ocular trauma related to falling, whereas the prevalence between the gender, the need for hospitalization or surgery and subsequent visual deficit were evaluated, as well as the severity of the cases. Results: Thirty-three (63.5%) of 52 patients were from the female gender, over which 30.3% had need for surgery and 18.2% developed visual deficit and 19 (36.5%) were from the male gender where 42.1% needed surgery and 26.3% developed visual deficit. Conclusions: The study has shown a higher prevalence of cases in the female gender, although the severity was higher in the male gender.

ABSTRACT

Objetivo: describir la Función de Sensibilidad al Contraste (FSC) en pacientes con miopía, astigmatismo miópico, hipermetropía y astigmatismo hipermetrópico. Diseño del estudio: serie de casos, descriptivo. Métodos: se incluyeron sujetos con miopía, astigmatismo miópico, hipermetropía y astigmatismo hipermetrópico, candidatos para cirugía refractiva y evaluados en la Unidad de Cirugía Refractiva de la Clínica de Oftalmología de Cali, con agudeza visual mejor corregida (AVMC) >=20/20, menores de 50 años de edad y sin antecedente de cirugía refractiva. La función sensibilidad al contraste se midió con el sistema OPTEC® 6500 (Stereo Optical Co, Inc., Chicago, USA) en frecuencias espaciales de 1.5, 3.0, 6.0, 12 y 18 ciclos/grado en condiciones fotópicas (85 cd/m2) y mesópicas (3 cd/m2). Se aplicaron las prueba no paramétrica de Wilcoxon y de Mann-Whitney para comparar diferencias entre la FSC entre los grupos de miopía/astigmatismo miópico e hipermetropía/astigmatismo hipermetrópico en las diferentes frecuencias espaciales. Resultados: se incluyeron 188 ojos de 95 sujetos con miopía y astigmatismo miópico (promedio de edad = 30,6 ± 7,6 años, 51% hombres) y 121 ojos de 64 sujetos con hipermetropía y astigmatismo hipermetrópico (promedio de edad = 43 ± 6,1 años, 62 % mujeres). La sensibilidad al contraste en los pacientes de ambos grupos fue mayor significativamente en condiciones fotópicas que en condiciones mesópicas, excepto en la frecuencia espacial de 3 ciclos/grado del grupo de miopía/astigmatismo miópico. Se encontró diferencia estadísticamente significativa del valor de la FSC entre los grupos de miopía/astigmatismo miópico e hipermetropía/astigmatismo hipermetrópico en condiciones fotópicas en casi todas las frecuencias espaciales, excepto en 3 ciclos/grado y en condiciones mesópicas en las frecuencias espaciales de 1.5, 3 y 6 ciclos/grado. Conclusiones: los valores de FSC son mayores en el grupo de pacientes con miopía y astigmatismo miópico que en los del grupo de hipermetropía y astigmatismo hipermetrópico. Estos valores podrían usarse como referencia en estos grupos de pacientes en nuestra población.


Purpose: to describe the contrast sensitivity function (CSF) in patients with myopia, hyperopia and astigmatism. Study design: descriptive case series study. Methods: subjects with myopia/myopic astigmatism and with hyperopia/hyperopic astigmatism looking for refractive surgery, and evaluated at the Unit of Refractive Surgery in the Clínica de Oftalmología de Cali were included. Their Best Spectacle Corrected Visual Acuity (BSCVA) was >=20/20, age less than 50 years and no previous refractive surgery. The CSF was measured using the OPTEC® 6500 (Stereo Optical Co, Inc, Chicago, USA) system for spatial frequencies 1.5, 3.0, 6.0, 12 and 18 cycles/degree in photopic (85 cd/m2) and mesopic (3 cd/m2) conditions. Statistical analysis was done using the Wilcoxon and Mann-Whitney tests for non-parametric samples, comparing the myopia/myopic astigmatism and the hyperopia /hyperopic astigmatism groups. Results: 188 eyes of 95 subjects with myopia/myopic astigmatism (age = 30,6 ± 7.6 years, 51% men) and 121 eyes of 64 subjects with hyperopia/hyperopic astigmatism (age = 43± 6.1 years, 62% women) were analyzed. The CSF was higher in both groups in photopic conditions except in 3 cycles per degree in myopia/myopic astigmatism group. Significant differences in the CSF were found between the two groups in photopic conditions excluding the 3 cycles per degree. In mesopic conditions diff erences were found for the spatial frequencies 1.5, 3 and 6 cycles per degree. Conclusions: The patients of the myopia/myopic astigmatism group had a contrast sensitivity function higher than those of hyperopia/hyperopic astigmatism group. These values could be used as reference for these patients in our population.

ABSTRACT

Objetivo: determinar el astigmatismo posquirúrgico en pacientes con diagnóstico de catarata pediátrica operados y con implante de lente intraocular plegable. Métodos: estudio observacional, descriptivo y transversal en 20 pacientes 28 ojos entre 2 y 14 años de edad, operados de enero de 2008 a septiembre de 2010 en el Instituto Cubano de Oftalmología Ramón Pando Ferrer. Se excluyeron los pacientes que presentaban alteraciones corneales. Se realizó lensectomía con capsulotomía posterior y vitrectomía anterior. Se implantó lente de acrílico hidrofóbico e hidrofílico plegable. Resultados: el 55 por ciento de los pacientes tenían entre 5 a 8 años de edad promedio 5,7 años. El tipo de catarata más frecuente fue la congénita 85 por ciento. La agudeza visual preoperatoria en el 60,7 por ciento de los casos era inferior a 0,3, a los 4 meses de operados 32,1 porciento tenían más de 0,9 y al año 35,7 por ciento alcanzó más de 0,9 y 28,6 por ciento entre 0,3 y 0,6. El astigmatismo antes de la cirugía en 32,2 por ciento estaba entre 0,5 y 1,5, a los 4 meses y al año fue entre 0,5 y 1,5 en 35,7 porciento. Conclusiones: el astigmatismo no sufrió variaciones con la realización de este tipo de cirugía. Una estrategia quirúrgica personalizada permite una mejor recuperación anatómica y funcional en estos pacientes


Objective: to determine the postsurgical astigmatism in patients operated on from pediatric cataract and with foldable intraocular lens implantation. Methods: an observational, descriptive and cross-sectional study of 20 patients 28 eyes between 2 and 14 years of age, who were operated on in the period of January 2008 through September 2010 at Ramon Pando Ferrer Cuban Institute of Ophthalmology. Some patients were excluded if they presented corneal alterations. Lensectomy, posterior capsulotomy and anterior vitrectomy were performed; hydrophobic and hydrophilic acrylic lenses were implanted. Results: fifty five percent of patients were 5 to 8 years of age average of 5.7 years. The more frequent type of cataract was the congenital 85percent. The initial visual acuity in 60.7 percent of cases was below 0.3; four months after surgery, 32.1 percent reached over 0.9 and after a year 35.7 percent reached more than 0.9, and 28.6 percent from 0.3 to 0.6. The astigmatism before the surgery in 32.2 percent of cases ranged 0.5 to 1.5, but four months and one year after surgery, it was 0.5 to 1.5 in 35.7 percent of patients. Conclusions: astigmatism did not vary with the performance of this type of surgery. A personalized surgical strategy allows achieving better anatomical and functional recovery of these patients

Since no study on the prevalence of refractive errors had been performed in this optometry clinic, this study was designed to determine the prevalence rate of refractive errors among the patients examined in this clinic in 2008/2009. In this cross-sectional study, the files of the patients examined in optometry clinic in 2008/2009 were reviewed. Variables included age, sex, rate and type of refractive errors and health of anterior and posterior eye segments. The prevalence rates of myopia, hypermetriopa, astigmatism and emmetropia were 11.83%, 15/38%, 45%, 27/34% respectively. The respective prevalence rates of refractive errors in men and women were: 10.3% and 13.1% for myopia, 16.7%, 15.1% for hypermetropia, 45.6% and 44.5% for astigmatism, and 27.4% and 27.3% for emmetropia. The mean ages of the patients with myopia, hypermetropia and astigmatism were 36.1 +/- 19.36, 41.27 +/- 22.06 and 32.16 +/- 20.98. Respectively Difference of the ratios of the patients with different ages suffering from refractive errors was significant [P<0.0001]. There was no significant difference in the male to female ratio in the patients with refractive errors [P<0.741]. This study showed that the prevalence rate of myopia increased up to the age of 20-30 years but that of hypermetropia decreased, and prevalence rates of myopia and hypermetropia in some age groups increased after the age of 40. In addition, the prevalence rates of refractive errors in different age groups were not similar and gender had no effect on the prevalence of refractive errors. These results are compatible with the results of other studies

ABSTRACT

Vision disorders are the fourth most common disability of children and the leading cause of handicapping conditions in childhood. The present study was undertaken to identify the prevalence and pattern of refractive errors among the school-entrant children at King Abdul Aziz Medical City [KAMC], Riyadh city - Kingdom of Saudi Arabia. The study population consisted of all the school-entrants [n = 1319], who attended the obligatory health examination for kindergarten and primary school entry during the period from March 2008 until October 2008. Every child was subjected to a 10min - visual acuity test [VAT] and autorefractive test applied by qualified optometrist. Children with a visual acuity of 20/28 or worse in one or both eyes, or with eye disorder [strabismus, nystagmus, ptosis and others] or abnormal ocular movement, were referred for a 45 min complete ophthalmic examination, which consists of: [1] Distance visual acuity [V/A], [2] cover - uncover test, and [3] non-cycloplpegic retinoscopy. Refractive error cut-off point was defined according to their spherical equivalent refractive error [SERE]. Out of the 1319 children, 60 children were diagnosed as having one or more refractive error, with an over-all prevalence of 4.5% [4.2% in boys and 4.9% in girls] with no sex difference. Prevalence of different refractive errors were as follows; Myopia [2.5%, 95%CI1.7:3.3%]

OBJETIVO: Determinar a prevalência da retinopatia da prematuridade em recém-nascidos pré-termo, associando-a com seus fatores de risco, além de comparar a incidência do vício refracional do tipo miopia e estrabismo entre as crianças pré-termo que desenvolveram a retinopatia da prematuridade (Grupo I) e as que não manifestaram esta retinopatia (Grupo II). MÉTODOS: Neste trabalho, empregou-se um estudo do tipo transversal, retrospectivo e documental dos prontuários de 147 crianças pré-termo, as quais foram atendidas no Ambulatório de Especialidades Jardim Peri-Peri, oriundas da Maternidade Mário Degni, nascidas no período de 7 de julho de 2004 a 10 de julho de 2008. O mapeamento de retina, com depressão escleral foi realizado, inicialmente entre 3 e 8 semanas de vida pós-natal e repetido a cada 1 a 4 semanas, dependendo dos achados fundoscópicos, até que a vascularização da retina se completasse ou a retinopatia da prematuridade se estabelecesse, e seguidos em consulta oftalmológica anual constituída por mapeamento de retina e exame refracional. Para a classificação da retinopatia da prematuridade foram utilizados os critérios da "International Classification of Retinopathy of Prematurity". Procedeu-se à análise do grupo total de prematuros, classificando-os em grupo I e grupo II, analisando a incidência de miopia e astigmatismo em cada grupo estudado. RESULTADOS: Nesta casuística, verificou-se a retinopatia da prematuridade em 35 crianças pré-termo (23 por cento), sendo que 112 crianças pré-termo (77 por cento) não desenvolveram esta retinopatia. No grupo I, 34 crianças (97 por cento) apresentaram-se no estágio 1 da retinopatia da prematuridade com regressão espontânea, e 1 criança (3 por cento) apresentou-se no estágio 2, evoluindo para doença limiar, tendo sido submetida ao tratamento cirúrgico por fotocoagulação a laser. Todos os recém-nascidos que manifestaram a retinopatia da prematuridade fizeram uso de oxigenoterapia, sendo a ventilação...


PURPOSE: To evaluate the prevalence of retinopathy of prematurity preterm newborn infants, associating it with its risk factors besides comparing the incidence of refractive errors like myopia and strabismus among preterm children who developed retinopathy of prematurity (Group I) and those who did not present this retinopathy (Group II). METHODS: A cross-sectional, retrospective and documental study of records of 147 preterm children who were examined at the "Ambulatório de especialidades Jardim Peri Peri" (Ambulatory of Specialties Jardim Peri Peri) was used in this paper. These children were born between July 7th, 2004 and July 10th, 2008 at "Maternidade Mário Degni" (Maternity Mário Degni). Fundus examination with escleral depression was first carried out between the 3rd and 8th weeks after birth and repeated every 1 to 4 weeks depending on the retinal imaging findings, until retina vascularization was complete or retinopathy of prematurity was present and it was followed by an annual ophthalmologic follow-up comprised of fundus examination and refraction. International Classification of Retinopathy of Prematurity criteria was used to classify the retinopathy of prematurity. Analysis of the whole group of preterm newborns was made and classified in group I and group II, and the incidence of Myopia and Astigmatism in each group was analyzed. RESULTS: This casuistry showed that retinopathy of prematurity was present in 35 preterm children (23 percent) and 112 preterm children (77 percent) did not develop this retinopathy. In Group I, 34 children (97 percent) were classified as stage I Retinopathy and had spontaneous regression and 1 child (3 percent) was in stage II Retinopathy and developed a threshold disease. CONCLUSIONS: In this study, the incidence of retinopathy of prematurity was similar to the international statistic data, since the threshold disease reached 3 percent of the cases. ABSTRACT

To determine the prevalence of refractive condition and its risk factors among students in Mashhad. A total of 2510 students representing a cross-sectional of the population of Mashhad were sampled using random cluster sampling strategy. Primary and middle school students underwent cycloplegic refraction. The refractive errors of high school students were measured using non-cycloplegic autorefraction. Myopia was defined as spherical equivalent [SE] of -0.5 diopter [D] or more, and hyperopia was defined as SE of +0.5 diopter [D] or more, and astigmatism of 0.75 cylinder diopter or greater. Examination was carried out in the school using standardized testing protocols. 2150 students [group 1: 1163 primary and middle school, group 2: 947 high school students and 13 missed data] participated. The prevalence of refractive errors in the 1st group was: myopia=2.4%, hyperopia=87.9%, astigmatism=9.8% and anisometropia=3.0% [SE difference at least 1.00 D], and in the 2nd group myopia=24.1%, hyperopia=8.4%, astigmatism=11.8% and anisometropia=5.6%. There was significant difference in refractive errors between girls and boys [P<0.001]. In primary and middle school prevalence of myopia increased with age [OR=1.3 95% CI: 1.03 to 1.7 and P=0.013]. The prevalence of refractive errors among students in Mashhad is high. Effective detection and treatment of these refractive errors is expected to reduce the incidence of amblyopia and strabismus and also can prevent substantive effects on academic performance

ABSTRACT

OBJETIVO: Verificar a prevalência do astigmatismo e a variação de seu eixo nas várias faixas etárias (2-99 anos) em uma população de uma clínica oftalmológica privada na cidade de Florianópolis, Santa Catarina, Brasil. MÉTODOS: Foi realizado um estudo retrospectivo, descritivo, transversal com base na análise de prontuários. Foi considerado astigmatismo uma diferença entre o maior e menor meridiano da córnea de -0,50 dioptrias(D) ou mais. Quanto ao eixo, consideramos: astigmatismo a favor da regra: entre 160º e 20º ; astigmatismo oblíquo: entre 21º e 69º e entre 111º e 159º; astigmatismo contra a regra: entre 70º e 110º. Os dados apresentados e analisados se referem à refração efetivamente prescrita para o paciente. RESULTADOS: Foram encontrados 20.866 olhos astigmatas (47,6% - Intervalo de Confiança IC 95% - 47,1-48). O eixo do astigmatismo se apresentou a favor da regra em 9574 olhos (45,9% IC 95% 45,2-46,6), oblíquo em 3893 olhos (18,7% IC 95% 18,1-19,2), e contra a regra em 7399 olhos (35,5% IC 95% 34,8-36,1). Verificou-se que a distribuição do astigmatismo oblíquo permanece linear no avançar da idade, enquanto há uma inversão do eixo do cilindro, de a favor para contra a regra com o tempo. CONCLUSÃO: Ficou bem evidenciada neste trabalho a marcante mudança do eixo do astigmatismo com o avançar da idade. Não encontramos relação entre o grau das ametropias e o eixo do astigmatismo.


PURPOSE: to determine the prevalence of astigmatism and the change of the axis according to the age in an ophthalmology private clinic population in the city of Florianópolis, Santa Catarina, Brazil. METHODS: A retrospective, descriptive, cross-sectional study was carried out based on the analyses of medical files. Astigmatism was classified as a cylindrical equivalent <-0,5 diopter (D). Astigmatism with the rule was defined as 160º to 20º, oblique astigmatism as 21º to 69º and 111º to 159º, and astigmatism against the rule when axes were located from 70º to 110º. The basis were presented and analyzed according to the glasses prescription. RESULTS: There were found 20866 astigmatic eyes (47,6% - 95% Confidence Interval CI - 47,1-48). The astigmatism axis was with the rule in 9574 eyes (45,9% CI 95% 45,2-46,6), oblique in 3893 eyes (18,7% CI 95% 18,1-19,2), and against the rule in 7399 eyes (35,5% CI 95% 34,8-36,1). It was verified the prevalence of the oblique axis remains stable with the age, while there is an inversion in the cylindric axis from with the rule to against the rule as time goes by. CONCLUSION: This study has made evident the change in the axis according to the age. It was not found any relation between the degrees of the ametropy and the axis of the astigmatism.

ABSTRACT

In Pakistan 11.4% of the blindness is due to uncorrected refractive errors. This study was carried out in two schools of Lahore to find the prevalence of refractive errors among high school children. It was an institution - based, cross sectional study involving the use of structured questionnaire and visual examination of the students. A total of 540 children studying in class 6th to 10th were examined over a period of two weeks. Vision of all the children was checked. The children with visual acuity less than 6/12 in one or both eyes, underwent objective retinoscopy and subjective refraction. The results revealed that 107 out of 540 [19.8%] of the children had refractive errors. Myopia was the most common refractive error being 43% [46/107] of the total. Astigmatism, both simple as well as mixed / compound was 35.5% [38/107 while hypermetropia was least common i.e. 21.5% [23/107]. Strong correlation was found between a positive family history of wearing glasses, watching television closely, close study, studying in dim light and over indulgence in computer or video games. It is recommended that adequate preschool examination of the children be made mandatory and part of the admission policy of all the schools in Pakistan. In addition, there should be periodic examination of the school children at least on annual basis

ABSTRACT

OBJETIVO: A prevalência do astigmatismo refracional e sua relação com a acuidade visual de resolução foram avaliados em amostra de crianças pré-verbais. MÉTODOS: Foram submetidas a exame oftalmológico 482 crianças normais com idades variando entre 2 e 36 meses. Após exclusão da amostra de 14 crianças com evidências de doenças oculares que pudessem alterar a acuidade visual, permaneceram 468 crianças (936 olhos), sendo 230 (49 por cento) do sexo masculino e 238 (51 por cento) do feminino. Todas foram submetidas a exame de acuidade visual binocular e monocular pelo procedimento do olhar preferencial através do método dos cartões de acuidade de Teller. Foi realizado em todas as crianças exame de refração sob cicloplegia e fundoscopia com oftalmoscópio indireto. RESULTADOS: O astigmatismo foi encontrado em 222 (47,43 por cento), crianças da amostra, com predomínio do tipo hipermetrópico composto e da forma a favor da regra em todas as faixas etárias. Em relação à magnitude, esta ametropia foi maior ou igual a 1,00 dioptria cilíndrica em 24,35 por cento dos sujeitos, maior ou igual a 1,00 dioptria cilíndrica em 24,35 por cento dos sujeitos, maior ou igual a 2,00 dioptrias cilíndricas em 5,55 por cento, menor que 1,00 dioptria cilíndrica em 26,92 por cento e maior ou igual a 1,00 dioptria cilíndrica e menor que 2,00 dioptrias cilíndricas em 18,73 por cento. A acuidade visual esteve dentro da faixa de normalidade em 219 crianças da amostra, independentemente da magnitude, tipo e orientação do astigmatismo. CONCLUSÃO: A acuidade visual de resolução medida pelo método dos cartões de acuidade de Teller não foi influenciada pelo astigmatismo.

ABSTRACT

OBJETIVO: Determinar a prevalência das ametropias em estudantes das redes pública e privada de Natal-RN. MÉTODOS: Foi realizada refratometria de 2.048 olhos de estudantes matriculados no ano letivo de 2001 e os dados avaliados com planilha do SPSS Data Editor 11. As ametropias foram divididas em: 1- de 0,1 até 0,99D (dioptria); 2- 1,0 até 2,99D; 3- 3,00 até 5,99D e 4- 6D ou maior. O astigmatismo foi reagrupado em I- a favor da regra (eixo entre 0 a 30 e 150 a 180 graus), II- contra a regra (eixo entre 60 e 120 graus) e III- oblíquo (eixo entre >30 e <60 e >120 e <150 graus). A faixa etária foi categorizada em 1- 5 a 10 anos, 2- 11 a 15 anos, 3- 16 a 20 anos, 4- 21anos ou mais. RESULTADOS: Dos erros refrativos, a hipermetropia foi o mais comum com 71%, em seguida astigmatismo, 34% e miopia, 13,3%. 48,5% dos míopes e 34,1% dos hipermétropes tinham astigmatismo. De acordo com as dioptrias, 58,1% dos míopes estão no grupo 1, 39% distribuídos entre os grupos 2 e 3. Os hipermétropes enquadram-se em sua maioria no grupo 1 (61,7%) e o astigmatismo no mesmo grupo com 70,6%. A associação dos eixos do astigmatismo dos dois olhos mostrou 95,2% com eixo a favor da regra nos dois olhos, diminuindo a porcentagem para os do eixo contra a regra (82,1%) e menor ainda para os do eixo oblíquo, apenas 50%. CONCLUSAO: Os resultados encontrados mostraram discordância com a maioria dos trabalhos internacionais, principalmente os orientais, que apontam a miopia como o erro refrativo mais comum e corrobora os nacionais, com a grande parte sendo hipermétropes.

ABSTRACT

PURPOSE: Postoperative astigmatism following intracapsular cataract extraction with or without anterior chamber intraocular lens implantation is reported as an outcome from a randomised controlled trial. METHODS: Five hundred and two of 1002 eyes randomised to intracapsular cataract extraction with anterior chamber intraocular lens (ICCE/AC IOL) and 417 of 998 eyes to intracapsular cataract extraction with aphakic spectacles (ICCE/AS) were seen for objective refraction one year after surgery. The prevalence and axis of astigmatism were evaluated using univariate analysis. Logistic regression was used to compare the postoperative astigmatism between the groups. RESULTS: Acceptable astigmatism (-0.5 to 0.0 DCyl) in the AC IOL group was found in 60 (12.0%) patients (95% CI 9.1%-14.9%) and in the aphakic spectacles group (AS) in 69 (16.5%) patients (95% CI 12.9%-20.1%), moderate astigmatism (-1.0 to-1.5 DCyl) was found in 153 (30.4%) patients (95% CI 26.4-34.6%) in ACIOL group and in 288 (69.1%) patients (95% CI 64.6%-73.6%) in AS group; and large astigmatism (-2.0 to - 8.0 D Cyl) was found in 289 (57.6%) patients (95% CI 53.1%-61.6%) in ACIOL group and in 60 (14.4%) patients (95% CI 11.0% 17.8%) in AS group. Large astigmatism was approximately four times more common in the ICCE/AC IOL group compared to ICCE/AS group. In both groups, most patients had "against-the-rule" astigmatism, 446 (88.8%) (95%CI 86.0%-91.6%) in AC IOL group and 348 (83.5%) (95%CI 79.9%-87.1%) in AS group. CONCLUSION: Astigmatism is common after intracapsular cataract extraction. Insertion of an anterior chamber IOL increases the risk of astigmatism.

ABSTRACT

Objetivo: Avaliar o perfil epidemiológico dos casos de ametropias atendidos no Núcleo do Hospital Universitário (NHU) da UFMS, já que as pesquisas em oftalmologia nessa área são extremamente pobras em nossa região. Métodos: Realizou-se levantamento epidemiológico retrospectivo na Seção de Arquivos Médicos (SAME), através da aplicação de protocolo de investigação de 2361 prontuários de pacientes com algum tipo de ametropia atendidos no NHU entre 1996 e 1998, contendo sexo, idade e tipo de ametropia (miopia, hipermetropia, astigmatismo ou presbiopia). Os dados foram analisados e discutidos. Resultados: Prevalência do sexo feminino (60 por cento) em todas as ametropias, o maior contingente atendido foi de pacientes com presbiopia (987 casos), seguida pela hipermetropia (701), miopia (434) e entre 20 e 29 anos, a hipermetropia entre 0 e 9 anos e os astigmatas entre 10 e 39 anos, enquanto a maior incidência de presbiopia foi na faixa etária acima de 40 anos.

ABSTRACT

Background: diversos métodos säo empregados para reconhecer e triar crianças menores de 3 anos de idade que apresentam fatores causadores de ambliopia e, entre eles encontra-se a fotorefraçäo. Assim como a fotorefraçäo a retinoscopia a dois metros fornece uma alternativa viável para a avaliaçäo da simetria dos reflexos de fundo de olho, com a vantagem de ser um método pouco dispendioso, rápido e facilmente aplicável por um examinador sem experiência prévia em retinocospia. Objetivos: visando avaliar este método, efetuamos um estudo em crianças de 6 a 36 meses de idade, frequentadoras de creches do município de Curitiba. De uma populaçäo de 1445 crianças avaliadas pela retinoscopia a dois metros, 263 crianças foram sorteadas para serem examinadas por um oftalmologista que procurou definir as crianças portadoras de fatores causadores de ambliopia. Os resultados dos dois exames foram comparados através de métodos estatísticos. Resultados: das 50 crianças triadas positivas pelo método da retinoscopia a 2 metros, 40 apresentaram defeitos compatíveis com o aparecimento de ambliopia. As outras crianças foram consideradas falso-positivas. Apenas 3 crianças declaradas livres de alteraçöes no exame de retinoscopia a 2 metros apresentaram anormalidades ao exame oftalmológico. A sensibilidade e especificidade do exame foram, respectivamente, 80 e98 por cento. Conclusöes: o exame de retinoscopia a 2 metros demonstrou, em teste realizado com grande número de crianças, ser um instrumento eficaz no combate a causas de ambliopia em crianças entre 6 e 36 meses, principalmente as de origem refracional e em especial a anisometropia

ABSTRACT

Fifty preterm neonates were followed up at the age of 6 months and 1 year. In addition to developmental assessment, a complete ophthalmological examination was done on both visits. The largest (62%) gestational age group was of 34-36 weeks. At 6 months, none of the infants had normal vision. At 1 year of age, 64% of the babies had normal vision while incidence of myopia and hypermetropia was 16% and 20%, respectively. There was an inverse relationship noted between gestation and incidence of refractive errors. It was also noted that with decreasing weight, the incidence of myopia increased. Myopia was seen exclusively among infants of birth weight of 2000 g or less. Birth weight had a significant positive correlation with astigmatism. No correlation of asphyxia with refractive errors was observed. It is recommended that all preterm babies should have an ophthalmological examination at one year of age with follow up later on.

ABSTRACT

A single center, single surgeon, nonrandomized, prospective clinical trial was performed comparing the keratometric induced astigmatism by different types of incisions and closures, one day, one week, one month and three months after 255 cataract surgeries. After phacoemulsification, or in rare cases after extracapsular cataract extraction, through a scleral pocket or limbal incision, patients received either a 5 x 6 mm, 5.5 mm, or 6 mm diameter polymethylmethacrylate optic posterior chamber intraocular lens. Incision was closed with either none, one horizontal or radial, or running 10-0 nylon suture. Vector analysis calculations of prism diopters of mean postoperative-induced keratometric astigmatism showed a trend to lesser values at each interval in scleral pocket incisions but statistically significant difference only at 1 day and 1 week between 9-mm and 6-mm limbal, and 5-mm and 6-mm scleral pocket incisions. Other subgroup analyses, including incision shape and distance from limbus, and type of suture, did not reach statistical significance. Horizontal, 5-mm, sutureless scleral tunnel incision showed less induced astigmatism with more rapid stable refraction

En desarrollo de una campana de salud visual se revisaron 336.354 ninos entre 5 y 14 anos de edad en el Departamento de Antioquia, durante los periodos escolares comprendidos entre octubre de 1986 y abril de 1988. Se motivo a la comunidad sobre los cuidados de la vision y sobre la importancia de identiificar a temprana edad y atender en forma oportuna los problemas oftalmologicos. Con el fin de lograr una cobertura total de la poblacion objeto se capacitaron 583 funcionarios del Gobierno Departamental y voluntarios de la comunidad, quienes a su vez se conviertieron en multplicadores de esa capacitacion en sus lugares de trabajo. Ademas se solucioaron los problemas de refraccion dotando de anteojos a los ninos que los necesitaran


A prevalence survey of ocular diseases in the school population aged 5 to 14 years, in Antioquia, Colombia, was carried out. Methodologies of Primary Health Care were employed; the target population was first examined by previously trained school teachers. Suspicious cases were referred to optometrists and, If necessary, to ophthalmologists. A total of 336.354 children were studied; in the primary screening 52.153 were found to have refraction problems; of them 12.233 were confirmed by optometrists and 7.556 required and received eye-glasses for correction
